On Lok Lifeways is expanding its senior healthcare and social services in Santa Clara County by partnering with Live Oak Adult Day Services, to serve seniors in Gilroy, Morgan Hill and San Martin. You’re invited to take a free tour of Live Oak’s facility in Gilroy.
On Lok Lifeways, provider of the Program of All Inclusive Care for the Elderly (PACE), and Live Oak Adult Day Services are partnering to bring PACE services to South County, which will empower seniors to keep living in their home. The new partnership will allow seniors to enjoy day activities at Live Oak’s site in Gilroy, while On Lok Lifeways San Jose PACE Center provides participants with adult day health care services.
